This online portal has been designed to assist in accessing data held by Essex Partnership NHS Foundation Trust.
Under the General Data Protection Regulation 2018, you have the right to apply for access to your records and in some cases, records of other people as an authorised representative. This is known as a Subject Access Request (SAR).
Patients and those authorised to act on the behalf of others can use this portal to submit a SAR.
A SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.
The Access to Health Records Act 1990 governs the right of access to deceased patient’s health records by specified persons, the act applies to records made after the 1st November 1991.

The AMS Request Management Portal is a secure encrypted solution, allowing third parties to submit SARs electronically and digitally via online forms.
Over 35 other NHS Trusts use the AMS portal for managing requests and disclosing records. The solution uses 256-bit AES encryption to protect your data and has UK data sovereignty.
AMS also hold ISO 27001 and Cyber Essentials certificates.
How to make a request
You can get a copy of your health records by creating an account through our online portal.
You can signup for a new account below or login if you have an existing account.
Requests are managed electronically and once complete you will be able to view your health record using the download function within the portal.
When making your request, please ensure the correct application type is selected. This will enable the team to process your request effectively.
Please make sure you have all necessary forms of ID or proof of entitlement before submitting the request. Identification checks are required by the Trust for security; this is to protect personal data from unauthorised access. Further guidance regarding the ID required can be found here.
You should receive access to your records within one calendar month (unless advised by court order) provided sufficient information to identify you is received with your application.
For any requests under Access to Health Records Act 1990 your request may take up to 40 days.
Please note that we are currently dealing with a significant backlog of cases and it is likely we will require a further two month extension to process your request through.
How do I request access to the health records of another person?
If you are an individual requesting records on behalf of a patient e.g. a family member, we will be unable to process requests without patient consent or proof of entitlement (i.e. Valid Power of Attorney).
If you are an organisation i.e. Solicitors, Police, Medical Agency, you should also use the online portal. Once registered on the site, you can use it repeatedly, manage multiple requests to us and download records at your convenience. This is generally quicker and an efficient way of receiving the information.
When making your request, please ensure the correct application type is selected. If you are applying to access the records of more than one person you must complete a separate application for each patient.
